Limitations of Conventional Gait Therapy

With conventional methods, providing highly intensive physiological gait rehabilitation to severely impaired patients is a huge challenge for health professionals today.

  • Highly labor intensive; two to three physical therapists to provide the necessary support
  • Related to high physical load for therapists
  • Limitations due to the physical capacity of the single therapist
  • Resource availability
  • Non-repetitive with many variables

Advantages of Lokomat Therapy

Effective Gait Training

Robot-assisted therapy enables effective and intensive training by providing a high number of task-specific repetitions, which is an essential factor underlying neuroplasticity and improved functional outcome.

Pooled data from a total of 70 clinical studies published in peer-reviewed journals including 39 RCTs, show that Lokomat training improves clinical outcomes related to walking abilities and functional mobility.(1)

Most Physiological Gait

The physiological gait pattern is ensured by the individually adjustable exoskeleton combined with the patented dynamic body weight support system.

The Lokomat provides natural weight bearing, afferent sensory feedback and physiological vertical displacement during the complete gait cycle. The hip and knee joint angles can also be individually adjusted during training to tailor to specific needs of patients.

Learn More >

Motivating patient challenge

During rehabilitation, patients need to be challenged in their present individual capabilties and beyond. Speed, body weight support, and robotic support can be adjusted to shape the intensity of the therapy and adapt to the patients’ individual needs.

Patients are motivated to reach their goals with various virtual reality based game-like exercises; also known as Augmented Performance Feedback (APF). Their level of activity influences the performance of the task and the gait pattern and the speed. Multiple guidelines suggest the combination of robot-assisted gait training with virtual reality to further improve lower limb motor outcomes.

Increased Efficiency

The Lokomat relieves the therapist from manually supporting the patient while walking and therefore allows therapists to focus on the patient and the actual therapy.

State-of-the-Art Gait Rehabilitation

Patients are well educated and informed about the most effective and efficient therapy options, and use this information to decide where to go for their rehabilitation.

A great reason to add the Lokomat to your therapy offering is that it has demonstrated to improve clinical outcomes related to walking abilities and functional mobility.

Success Stories

I Felt That it was Doing Something Good
“I couldn’t explain it, but I felt that it was doing something good,” he says. “I fell in love with the Lokomat that day!” After 16 therapy sessions, Phillip’s force measured by the LokomatPro sensors had improved greatly and he decided to move closer to the hospital in order to be able to continue the therapy with the Lokomat.

Giving up was not an Option
Gabriele was an active person and only 44 years old when she had a stroke which led to hemiparesis of her left side. "I led a very active life; I always had many goals and worked hard to achieve them. It took me a lot to get used to the new situation," explains Gabriele. But 12 years after the stroke, not much had changed and walking still required a lot of effort. Then, a friend told her about the Lokomat. "I find the training with the Lokomat very helpful, as I can work on my balance and I feel safer while walking," she says happily. "I can already walk much faster and roll my foot through the step." The progress she has made after two months of therapy has fueled her motivation. "It’s fantastic what I achieved with this training so far, even all these years after the stroke! I wish all stroke patients could have access to this therapy."

Step by Step Towards Long-Term Goals
Training with the LokomatPro is good because I have to be active and because it challenges me. There are new situations over and over again where I have to react differently. Training with the Lokomat is diverse and exciting because I can control the directions myself with the amount of strength I put in. And with the FreeD I have even more freedom of movement."

Towards an Independent Daily Routine through Play
M.S. was only 16 when he had a stroke in the left side of his brain resulting in a right-sided hemiparesis with a sensitivity disorder. After training with the LokomatPro FreeD four times a week for a mere three weeks, he stated: "I can stand on my own again and I feel much safer in my daily routine. I like the gamees best! The training is much more fun."

One Hundred Miles for the Best Therapy
"Carson came to us with strength and gait impairments," explains his therapist Jenny Schmit. Using a walker, Carson's legs criss-crossed and he was able to walk only very slowly. "He jumped into the Lokomat and had so much fun during the therapy," explains his mother, Carla. With games and fun exercises, the staff at Cincinnati Children's Hospital offers intensive and motivating therapy, and very soon, Carson started showing great improvements. "He was just flying! His mobility was better and he was so much more self-confident," says Carla happily. "I was just delighted at his improvement. Driving 100 miles is nothing when you want to get the best care for your child."

Effectiveness of Lokomat
Training Proven

More than 550 studies have been published on Lokomat therapy in peer-reviewed journals.

Gait training with the Lokomat leads to clinically relevant improvements in adult and pediatric patients with severe to moderate impairments in walking abilities and functional mobility in the following domains:

  1. Improvement in walking speed

Patients undergoing Lokomat training demonstrate clinically significant improvements in walking speed, as measured with the 10-meter-walking test.
(Wirz et al. 2005, Borggraefe et al. 2010, Paker et al. 2013, Ucar et al. 2014, Varoqui et al. 2014, Park and Chung 2018)

  1. Improvement in walking endurance

Walking endurance in patients with severe to moderate gait impairments, as measured with the 6-min-walking-test, increases above the minimal clinically important difference with Lokomat training.
(Wirz et al. 2005, Borggraefe et al. 2010, Alcobendas-Maestro et al. 2012, Schoenrath et al. 2015, Straudi et al. 2015, Beretta et al. 2020)

  1. Improvement in functional ambulation

“People who receive electromechanical-assisted gait training in combination with physiotherapy after stroke are more likely to achieve independent walking than people who receive gait training without these devices. We concluded that eight patients need to be treated to prevent one dependency in walking.”
(Mehrholz et al. 2020)

  1. Improvement in functional mobility

Training with Lokomat leads to improvements above the minimal clinically important difference in functional mobility, as measured with the Time-Up-and-Go.
(Wirz et al. 2005, Ucar et al. 2014, Varoqui et al. 2014, Bae et al. 2017, Park and Chung 2018)

  1. Improvement in balance

Robot assisted gait training with Lokomat leads to clinically significant improvements in static and dynamic balance as measured with the Berg Balance Scale in the intended patient population.
(Bang and Shin 2016, Han et al. 2016, Calabrò et al. 2017, Clerici et al. 2017, Park and Chung 2018, Yun et al. 2018, Kim et al. 2019)

  1. Improvement in cardiovascular fitness

Training with the Lokomat improves cardiovascular fitness (as measured with the peak oxygen consumption) in patients who are not yet independent ambulators.
(Chang et al. 2012, Stoller et al. 2015, Gorman et al. 2016, Han et al. 2016, Bae et al. 2017)

What Experts Say

Today, the first patient in the entire country tried the Lokomat® FreeD Pediatric. The young man worked incredibly hard, walked for 35 minutes, and loved every second of it. Seeing him strengthen his muscles while simultaneously having a blast with the games was amazing. At the end of the session, he asked me when he could come back and train again. What more can a PT ask for?

Achieve Best Possible Results With the Lokomat
"The Lokomat allows us to provide our patients with more intensive gait therapy. They experience more repetitions and are motivated by the Augmented Performance Feedback to actively contribute with maximum effort to their therapy. We are reminded daily how this intensity translates into an increase in functional mobility. The Lokomat technology allows us to provide patients with the opportunity for best possible outcomes."
